Actual Fukushima worker Happy11311 tweeted about the failure of the back-up coolant system in SFP4.

 

 

続き2:片側のポンプが停止したらバックアップポンプが起動するはずなんだけど、こっちも同じく電源供給されずに動かなかったみたい。簡単にいうと設計ミスとチェック漏れでし。根本的原因は調べてるみたいだけど湿気とか塩害の可能性もあるのかなぁ…。

The back-up pump was supposed to be started when one of the two pumps stoped, but that one was not supplied with power either, it didn’t work. In the simple put, it was the design error and lack of checking. They are investigating the cause seemingly but I wonder if humidity or salt damage are involved..

続き3:冷却系は事故の初期段階で突貫で据え付けた物が多いから設計時間もなく結構チェックしきれてないものが多いのかなぁ…。でもあれから一年近くたってるから、あってはいけない事でし。またトラブルが起きてからの対応で「転んでからの杖」になってしまったでし。
